Every system failure this week had the same fingerprint: the signal was present before the damage, clear enough to act on, received by no one who owned the outcome. That is not a communication problem. It is a Norman’s Gap problem.
WHY THIS, WHY NOW
The pattern that surfaced this week
Tuesday’s edition examined a pipe that ruptured because accountability was distributed across PRASA, municipal governments, federal reconstruction funds, the governor’s office, and the Puerto Rico Senate.
Thursday’s edition examined a congressional hearing where a sitting cabinet secretary was corrected twice, publicly, with a battery held up as evidence, and did not update the argument. One failure is structural. One is internal. Same gap.
Why it cannot wait
Organizations spend heavily on signal infrastructure: dashboards, escalation protocols, early warning systems, briefing cycles. They spend almost nothing on the ownership architecture required to receive those signals at the decision level. The gap between what a system knows and what a named owner acts on is widening. Another week without naming it is another week the deterioration accumulates.
What shifts if leaders see this clearly
The question changes. Instead of asking why a pipe burst or why a cabinet secretary got a fact wrong in a hearing, the leader asks: what signals exist in my organization right now that have no named owner at the reception point?
That question has an answer. Acting on it is the work.
THE DEEP DIVE
Puerto Rico’s Superaqueduct had been communicating for years.
The signal was present in maintenance reports, in the Manati rupture nine months prior, in Senate hearing testimony confirming visible failures since mid-2025.
Not hidden.
Received by an organizational structure that distributed accountability across enough entities that no single actor moved at the speed the pipe required.
By the time the rupture came on June 12, the signal had passed through PRASA leadership, municipal emergency coordinators, federal reconstruction oversight, and legislative inquiry. Each layer received a version of reality. None of them constituted ownership of the outcome.
Secretary Burgum’s testimony on May 13 ran the same mechanism at the human level.
Representative Huffman’s battery correction was not ambiguous. Direct, physical, public, on the record. The signal entered. The internal system filtered it before the response formed. Twenty minutes later, the same argument appeared again. The signal had not penetrated the decision architecture.
Not the organizational one. The internal one.
Norman’s Gap operating at two levels simultaneously. At the system level: the distance between what PRASA’s infrastructure was signaling and what the ownership structure could absorb and act on.
At the human level: the distance between what the secretary’s environment was transmitting and what his internal regulation allowed him to receive.
Same gap. Different substrate. Identical mechanism.
Both systems failed at the reception point. Not the transmission point. That distinction is the whole argument.
That’s what “reception point” means. It’s just: who’s standing there when the signal lands? In Puerto Rico, the answer was distributed across five agencies. In the hearing room, the answer was a leader whose internal state couldn’t take in information that threatened the argument he was already making.
THE SYSTEM PATTERN
What the pattern is
Signal reception fails not because the signal is absent, but because no regulated owner exists at the point where the signal arrives.
Where it repeats
In public infrastructure management, where asset deterioration accumulates across agencies with no single named owner for the outcome of prevention.
In executive decision-making, where threat response filters corrections before the prefrontal cortex can update the model.
In organizations that have built escalation systems without building ownership systems to receive what those escalations deliver.
Where leaders miss it
At the moment they mistake signal volume for signal reception. They assume that because the information exists, the system has processed it. Information without a regulated owner at the reception point is noise.
The gap is not in the data.

The gap in play
Both cases this week scored Pressure 8.5 with Regulation at or below 3.5.
Puerto Rico’s water system has been carrying that gap for decades. The congressional hearing closed that gap in twenty minutes, and the gap won.
What it predicts
Leaders who do not close the ownership gap at the reception point will keep receiving signals they cannot act on. The infrastructure degrades. The frame holds. The outcome locks.
What it demands
One named, regulated owner at the point where the signal arrives.
Not a team.
Not a department.
One name, one outcome, one threshold that triggers action before the Norman Failure Condition is met.
MOS ARCHITECTURE
The structural correction this week requires is ownership mapping at the reception layer, not the transmission layer.
Most MOS implementations build escalation discipline and KPI truth.
Fewer build reception ownership: the assignment of a named individual whose job is not to relay the signal but to own the outcome if the signal goes unanswered.
Puerto Rico had no such owner for pipe deterioration.
The secretary had no structural protocol requiring his frame to be tested against market data before the policy position locked.
Both systems needed a reception owner. Neither had one.
THE INNER OPERATING SYSTEM (IOS)
The internal shift this week requires is the willingness to be wrong before an audience.
Both cases failed at this threshold.
PRASA leadership separated infrastructure failure from management accountability in public testimony.
The secretary constructed a rebuttal before the correction had been received.
The regulated leader does one thing differently:
stops.
receives
updates.
That sequence is only available if the internal system is not in threat response at the moment the contradicting signal arrives. Regulation before rebuttal.
That is the IOS correction both cases demanded.
