THE SIGNAL
The Lincoln Memorial Reflecting Pool got a $14 to 16 million renovation. However, within days of being refilled, a significant algae bloom appeared. Congress is now involved, looking at the contracting process, the cost escalation, and why a freshly completed project failed this fast.
Aquatic scientists needed no investigation to explain it. They know shallow water, nutrients and a dark coating can mean trouble.
I live in Florida, my pool gets to around 84 to 86 degrees inn the summer, we know growth can happen fast, why didn’t they see this?
Those conditions are well known. They were there before the renovation began, when the coating was selected, when they started the refill. The system failed on day one because nobody confirmed all of this before going-live.
THE FAILURE POINT
The algae is not the failure. It is the outcome.
The real failure happened before the algae ever appeared. Someone decided the project was ready because construction was finished, not because anyone proved it would actually work under real operating conditions. Somewhere in this whole chain, a system was declared ready because the work was done, not because the conditions for success had been confirmed.
The coating was completed.
The pool was refilled.
The ribbon was cut after construction completion, not on proof that the completed work would hold under real world conditions.
That validation step, the one requiring an owner to sign off on the conditions to ensure success, before water went in.
SIGNAL WITHIN THE SIGNAL
This is Signal Compression moving through a procurement and engineering review chain. To me, this is Signal Compression. The information that mattered never reached the people making the final decision.
The physical conditions never changed. The pool remained shallow, nutrients were available, and the new dark coating absorbed additional heat. From a biological perspective, algae wasn't surprising, it was predictable. The signal was biological and physical:

That signal was known to engineering before the contract was signed. It could have been spotted inside every discussion between aquatic scientists and the authorization to refill. What reached the decision level was a construction milestone.
BEHAVIOR UNDER PRESSURE
We see this anytime work is done, completion means its okay to run.
Under the schedule and the budget pressure, most organizations will treat finished construction as the problem is solved.
A question that should have been asked, will the completed system perform under real operating conditions?
We see this in every area where systems are handed from builders to operators without an operating and safety check.
Builders certify completion.
Operators inherit assumptions.
When the system fails on the first operating cycle, who was checking the conditions? The conditions were present and known before the handoff. Nobody named them because naming them would have gone against the pressure for results.
SYSTEM DRIVER - MOS
The correction is what we see in an engineering change. When I worked in aerospace, engineering changes didn't move because someone said they were finished. Every change had to pass through a series of approval gates, and every engineer signed their name before it moved forward. There was clear ownership. Not a checklist one person reviewed. But a separate sign-off that ensures someone reviewed and is saying this system will perform as designed under real world conditions?
For the Reflecting Pool, that gate would have poked at what happens to water with all these variables.
The answer was available. There was no gate process to require it. This fix sounds simple, no system goes live after a major renovation without an individual signing off on it. How about adding four words to every project close-out: operational conditions confirmed by.
LEADER DRIVER - INTERNAL OPERATING SYSTEM (IOS) - REGULATE
The internal failure is where a leader accepts completion as confirmation.
Under project pressure, the brain looks for permission to close the loop. A finished milestone gives it. This is where a regulated leader slows down instead of speeding up. Before accepting "it's finished," they ask one more question: Have we actually proven this system will work where it's going to live?
That question is uncomfortable after a $14 to 16 million renovation because it implies the work may not be done.
The regulated leader asks it anyway.
IF YOU DO ONE THING TODAY
As a Leader, you have plenty of initiatives, identify one in your organization that completed an implementation in the last 12 months.
Ask did it go through an operational gate separate from the construction or build completion.
If the answer is no, or if no one can produce the document, you have a potential embarrassment on your hands.
Find an owner.
Test the system before the first failure, not after.
